オパガニブの第2相/第3相試験は、広範かつ厳密なデータを短期間で収集するために、世界各地の最大40の試験実施医療機関で最大270人の被験者を登録することになっています。この試験に対応するため、RedHill社はVeeva Vault CDMSを採用しました。Vault CDMSは、電子データ収集(EDC)、コーディング、データクリーニング、レポート作成のための最新クラウドプラットフォームで、臨床試験の構築および展開プロセスをスリム化して効率化する、柔軟なEDCをRedHill社に提供します。データにフォーカスしたグローバルな医薬品開発業務受託機関(CRO)であり、Veevaの認定パートナーであるBioforum社は、長年のクライアント企業であるRedHill社により、今回のCOVID-19臨床試験におけるVault CDMSの実装および設定担当として選ばれています。

オパガニブ(ABC294640、Yeliva®)について

新規化学物質「オパガニブ」は、抗がん、抗炎症、および抗ウイルス活性を示す独自開発のファーストインクラスの経口投与スフィンゴシンキナーゼ2(SK2)選択的阻害剤であり、さまざまながん腫、ウイルス性疾患、炎症性疾患、および消化器疾患をターゲットにしています。SK2を阻害することにより、がん増殖、ウイルス複製、および病的炎症に関連する複数の細胞性の経路に影響を与えます。

オパガニブは、米国食品医薬品局(FDA)から胆管がん治療用のオーファンドラッグ指定を受けており、進行性胆管がんに対する前期第2相試験および前立腺がんに対する第2相試験で評価が行われています。また、新型コロナウイルス感染症(COVID-19)に対する国際共同第2相/第3相試験および米国における第2相試験でも評価が進められています。

オパガニブは前クリニカルデータにて抗炎症活性と抗ウイルス活性を示しており、肺炎などの肺炎症性疾患や肺線維症を軽減する可能性があります。オパガニブは、COVID-19の原因ウイルスであるSARS-CoV-2に対して強力な抗ウイルス活性を示し、ヒト肺気管支組織のin vitroモデルではウイルスの複製を完全に阻害しました。さらに、気管支肺胞洗浄液中のIL-6およびTNF-alphaレベルを低下させることにより、インフルエンザウイルス感染による致死率を低下させるほか、緑膿菌誘発肺障害を改善することが前臨床のin vivo試験[2]で明らかになっています。

オパガニブは、もとは米国に本社を置くApogee Biotechnology社によって開発されたもので、がん増殖、炎症系、消化器系、および放射線防護の各種モデルに対する前臨床試験に加え、進行性の固形がんを有するがん患者を対象とする第1相試験にも成功しています。

コンパッショネート・ユース制度の下、イスラエルの先進病院においてオパガニブによるCOVID-19患者(WHOの順序尺度に基づく)の治療が行われました。先頃、オパガニブによるこれらのCOVID-19重症患者の初めての治療データが公開されました。[3] 治療成果を分析した結果、同病院で遡及的に調べた症例対照群との比較において、コンパッショネート・ユース制度の下でオパガニブによって治療した患者では、臨床転帰と炎症マーカーの両面で非常に有効であることが示唆されました。オパガニブ投与群の患者は全員が人工呼吸器の必要なく退院したのに対し、症例対照群は33%の患者に人工呼吸器が必要でした。また、高流量式鼻カニューレ離脱までの中央値は、対照群の15日に対し、オパガニブ投与群では10日に短縮されました。

オパガニブの開発は、国立がん研究所(NCI)、生物医学先端研究開発局(BARDA)、国防総省、FDA希少疾病用医薬品開発室(OOPD)などの米国連邦政府および州政府機関からApogee Biotechnology Corp社に授与された補助金や契約によって支えられています。
